The Workers' Compensation Commission recently published an updated 30c form for filing a workplace injury or illness.
The new form contains a checkbox to indicate a Firefighter Cancer Claim under Section 7-313p of the CT General Statues. Members filing claims under our cancer relief program should check this box prior to submitting to make sure their claim is filed correctly.
The new form is attached to this email but it can also be found on the UPFFA website under the Workers Comp section or on the WCC website.
